Community
Health Grants Program
The Peninsula Health Care District was formed to promote and support
the health care services for its residents and others served by
the District. Its Mission includes performing duties as the lessor
of Peninsula Hospital as well as responding to the local health
needs and allocating resources for programs that enhance the health
of the community.
The Peninsula Health Care District aims to achieve measurable improvements
in the health of its community and the Community Health
Grants Program is the cornerstone of this effort. Each
year the ad hoc Service Agreement Committee reviews the health needs and priorities
of the community within the District’s boundaries and as well
as the funding available for the grants program. The grants program
funds are used to support nonprofit community-based agencies, programs,
and services that are reflective of, and responsive to, the identified
priority needs within the community.
Purpose of the Community Health Grants Program
The purpose of the Peninsula Health Care District’s Community
Health Grants Program is to invest in nonprofit, community focused
programs, services, and interventions that serve to improve the
health status of district residents. The District uses a comprehensive
definition of health to include those behaviors, programs, activities,
and support that promote and protect the physical, psychological,
and social well-being of an individual, a family, and a community.
