YFES Crisis Intervention & Suicide Prevention
Youth and Family Enrichment Services (YFES) crisis line was one of the first in the country, founded by one of the pioneers of the crisis movement, Charlotte Ross, in 1966. Many things have changed since its inception over 35 years ago, but the program’s primary function remains the same: to provide immediate, 24-hour assistance to people in times of great stress or grief. Additional services include: suicide education, survivors of suicide group, and youth intervention team. YFES believes that crises are a normal part of life, and that sometimes these crises can be too large for us to handle on our own.

Millbrae After School Nutritios Snack Program
The Millbrae Homework Center is open weekdays from 3 to 4pm and povides consistane, supervised, and academically focused learning opportunities for all students. The Programs hope to serve a minimum of 220 students after school snacks. The Homework Center staff will teach nutritional food components to students who utilize the Youth Center. The prinmary goal is to serve more middle school students, to increase the number of students to recieve academic and social support and to promote students' healthy eating habits and general well being incliding socialization and physical activity.

Mills-Peninsula Senior Focus Adult Alzheimer’s Day Health Care
Senior Focus offers the only specialized and separate day health care setting for people with Alzheimer’s and other forms of dementia in San Mateo County. Staff is specially trained in dementia care and provides therapeutic activities, assistance with personal care, nursing care, physical, occupational and speech therapies, hot meals and snacks. Consulting services for family caregivers include help with managing difficult behaviors, home safety and other issues, and a regular caregiver support group.

Mills-Peninsula Senior Focus Wise and Well Programs
For 11 years the Wise and Well program has been providing monthly health screenings and services through five senior centers located throughout San Mateo County. Wise and Well is a cooperative effort involving the Peninsula Health Care District, San Mateo County Aging and Adult Services and the health care resources offered by Mills-Peninsula Senior Focus. The program focuses on preventing chronic conditions such as diabetes and cardiovascular disease and improving older adult quality of life through counseling, multilingual health education and health screenings.

Women’s Recovery Center for Tracey’s Place of Hope
The Women’s Recovery Center operates five residential recovery facilities and one outpatient-counseling center for women, children, and families. Tracey’s Place of Hope hosts 6 beds for women age 18 or under.

Ombudsman Program of San Mateo County
The mission of the Ombudsman Program of San Mateo County is to promote standards of excellence in advocacy and enhancement of the quality of life for residents of long-term care facilities in the county. Services provided include: advocacy on behalf of facility residents, investigation of elder abuse that occurs in the facilities, working with families to find suitable placement for their loved ones and assistance in accessing MediCal.

Children’s Health Initiative, County of San Mateo
San Mateo’s Children's Health Initiative provides all county children with access to health insurance. San Mateo County uses a three-option model for health insurance, with coverage for families up to 400 percent of the Federal Poverty Level, which is still below the county's estimated cost to raise a family.

College of San Mateo Nursing Program
The College of San Mateo Nursing Program provides students with opportunities for learning at the College, local hospitals and community health agencies. The program is fully accredited by the California Board of Registered Nursing. Upon graduation, the candidate receives an Associate in Science Degree in Nursing, and is eligible to take the National Council Licensing Exam (NCLEX) examination. The graduate is also eligible to transfer to programs offering a Bachelors degree in nursing.

Hospital Consortium - Stroke Awareness - Public Campaign
The mission of the Hospital Consortium of San Mateo County is to enable the member hospitals to work together wherever legally permissible towards developing and enhancing a collaborative county network designed to improve the delivery of health care and the general health of the people of San Mateo County. Stroke is the number three killer in San Mateo County and in the country. Our opportunity is to educate our senior community that STROKE IS TREATABLE. Shelter Network
Shelter Network was established in 1987 to address the growing problem of homelessnes on the Peninsula. Through seven programs located throughtout San Mateo County, Shelter Network provides short-term and longer-term transitional housing for homeless families and individuals, and permanent supportive housing for chronically homesless adults. THe Wellness Project will increase the parenting information and skills of parents resding in Shelter Network's family shelters and educate parents concerning nutrition and hygiene.

Stanford's Fairwell to Falls Program
The Farewell to Falls Program is an injury prevention program aimed at reducing the recidivism rate of falls for older adults. As it is estimated that more than half of older adults who fall will fall again, targeting the senior who has fallen can have a significant impact by helping the older adults who has fallen can have a significant impact by helping the oder adult maintain his/her independance.

Jewish Family and Children's Services
Jewish Family and Children's Services is being honered with a three-year grant which is used to provide non-medical, non-Medicare covered, in-home assistance for basic services. The goal is to expedite hospital discharges, increase independence, prevent unnecessary re-hospitalizations, as well as, provide a wide range of social services, including "end of life" planning. Each year's grant provides services for 50 seniors living within the District.

Health Plan of S.M. Co. - Obisity prevention in the Latino Community
HPSM and the San Mateo County Department of Public Health have established a strong relationship and collaboration in creating healthy communities within San Mateo County. Efforts to improve the health of children have been at the forefront of this longstanding collaboration. The Shapedown program was developed by a nutritionist at UCSF, and has been shown in multiple medical studies to be a successful way for children, teens and their families to make the difficult lifestyle changes necessary to lose weight and maintain weight loss.
