The Peninsula Health Care District invites proposals for its
2010-2011 cycle of Community Health Investment Program

The Peninsula Health Care District was formed to promote and support the health care services for its residents and others served by the District.  Its mission to ensure Peninsula Medical Center provides needed core services, to support programs that share our vision, and to do so in collaboration with other providers and qualifed members of our community.

Based on its current budget, available resources, and criteria, the Board of Directors will allocate funds to support nonprofit community based agencies, programs, and services, which improve the health status of residents of the Peninsula Health Care District.  It is the District’s aim through
the grants program to achieve measurable improvements in the health of the community.

Community Health Investment Committee
To assist the Board in carrying out its grants process in a focused, thorough, and sound manner, the Board has established its' Community Health Investment Committee. The members review every proposal and advise the Board.

Eligibility
The Community Health Investment Program will support 501(c)(3) programs and services, which directly serve residents of the Peninsula Health Care District.

Program Priorities
The District Board uses a comprehensive definition of healing behaviors, programs, activities, and interventions in determining if requests promote health and protect the physical, psychological, and social well-being of an individual, a family and a community.

The District Board has identified several broad program areas within which the Community Health Investment funding will be allocated during each fiscal year.  The District aims to support programs, organizations, and community collaboratives with demonstrated competencies in priority areas and with the greatest potential for achieving measurable results in those areas.  Current health priorities are: primary care access for those in need, childhood obesity and nutrition, health education for risk reduction, senior services, recruitment and training of healthcare professionals.

The Board's Strategic Plan goals and objectives, as well as, the healthcare priorities identified in the San Mateo County Community Needs Assessment will serve as critical indicators in evaluating and awarding grants.

Grantmaking Policies and Guidelines
Grants will generally be made in the range of $10,000 - $50,000 annually.  Agencies who have historically received funding at higher level from the District may make application for awards consistent with previous support.

Proposals, which are specific with respect to outcomes, programmatic milestones, interventions, time frames, funding sources, sustainability, evaluation methods, and accountability, are of highest interest to the District.

The District is supportive of programs which will use Community Health Investment funds to leverage additional funds or other support to amplify the impact of the District’s resources.

The District encourages results-based community collaborations among agencies, local government and the private sector, where appropriate, to improve the health of District residents.  In addition, the District is interested in working collaboratively in effective, program-focused efforts with other local funders and with local, regional, and national healthcare initiatives.

The Community Health Investment Program will support the services, programs, agencies with strong, successful track records and the highest probability of meeting their defined goals and objectives.

Grants made by the Peninsula Health Care District must directly and, in most cases, benefit those who reside in the District.  Agencies who provide services to individuals referred from outside the District, must be able to demonstrate that the use of grant resources meet and are applied to local needs.

The District will generally not consider requests for general institutional operating support, requests to cover budget deficits, or requests to support research projects.

Recipients are required to provide regular financial and operational reports and periodically may be requested to make presentations to the Community Health Investment Committee and/or the Full Board.

Application Process
Nonprofit agencies interested in applying for the Community Healthcare Investment funding grant should submit a letter of intent describing their proposed project. 

The Community Health Investment Committee will review the Letters of Intent and Full Applications. The Committee will be advisory to the Board and will present recommendations at the Board's December 10, 2010 meeting. Committee members will include:

2 - Directors from the District Board; one serving as committee chair
2 - Community Healthcare Providers and/or funders
2 - District Community members-at-large
District Chief Executive Officer

The committee will carefully review all letters and will invite full proposals from agencies whose requests are consistent with the priorities, policies, and guidelines of the grants program.  The committee in a competitive and objective process will review full proposals.

Timetable:

October 8, 2010 Letters of Intent are due / 1-2 pages maximum.
October 21, 2010 Invitations for full proposals will be issued by District
November 19, 2010 Full Proposals are due in District office by
5:00 PM
December 10, 2010 Grants will be announced
Guidelines for Letters of Intent
Letters of intent should be addressed to the Community Health Investment Committee Chair, Peninsula Health Care District and mailed to the address given below.  The letter should be no longer than two (2) pages and should include the following information:
  1. Name of the applicant agency (or collaborative agencies) with contact person, phone, fax, address and email address
  2. Description of need, problem, or opportunity to be addressed
  3. Statement of specific program objectives and intended program outcomes
  4. Overview of program approach including key assumptions underlying program planning
  5. Explanation of the proposed program's consistency with the Community Health Investment Program priorities, policies, and guidelines.
  6. Description of the applicant's agency or collaborative including mission statement and explanation of the applicant's capacity to carry out the proposed program
  7. The funding amount requested from the District's program
  8. Summary of the proposed project budget

Letters of intent will be carefully reviewed and evaluated on the basis of two consideration:

  • Responsiveness to the Peninsula Health Care District's purpose and program priorities for the current fiscal year.
  • Consistency with the guidelines and principles of the Community Health Grants Program
