The Peninsula Health Care District invites proposals for its
2011 - 2012 cycle of Community Health Investment Grants

The Peninsula Health Care District was formed to promote and support health services for the residents and others served by the District.  Its mission is to ensure Mills Peninsula Medical Center maintains the provisim of core services; to support programs that share our vision, and do so in collaboration with other providers and qualifed members of our community.

Based on its current budget, health priorities, and program criteria, the District's Board of Directors will award funds to support nonprofit community based agencies, programs, and services, which improve the health status of our residents.  It is the District’s aim through the grants program to achieve measurable improvements in the health of the community.

Community Health Investment Committee
The Committee was established by the Board to assist in carrying out the grants process in a focused and thorough manner. Committee members are appointed by the Board Chair and serve 3-year terms. Committee members include:

2 - Directors from the District Board; one serving as committee chair
2 - Community healthcare providers and leaders
2 - District community members-at-large
District Chief Executive Officer

The members advise on health priorities, review all Letters of Intent, determine which agencies will be asked to submit Full Proposals, review all submissions, participate in site visits, and make recommendations for funding to the Board at its December meeting.

Eligibility
The Community Health Investment Program will support programs and services which directly serve residents of the Peninsula Health Care District. Non Profit organizations with tax exempt status 501(c)(3) of the Internal Revenue Code are eligable to apply for District grants.

Program Priorities

The District Board has identified several broad areas of health care need within the District. The District aims to support programs, organizations, and community collaboratives with demonstrated competencies in achieving measurable results that address the Board's health priorities. These are: primary care access for those in need, childhood obesity and nutrition, health education to increase personal responsibility and improve behaviors to reduce health risks, senior services, and recruitment and training of healthcare professionals.

Grantmaking Policies and Guidelines
Grants will generally be made in the range of $10,000 - $50,000 annually.  Agencies that can demonstrate large numbers served and have a track record of improved access and/or health status may apply for larger grants.

Proposals, which are specific with respect to outcomes, programmatic milestones, interventions, time frames, funding sources, sustainability, evaluation methods, and accountability, are of highest interest to the District.

The District is supportive of programs which will use Community Health Investment funds to leverage additional funds or other support to amplify the impact of the District’s resources.

The District encourages results-based community collaborations among agencies, local government and the private sector to improve the health of District residents.  In addition, the District is interested in working collaboratively in effective, program-focused efforts with other local funders and with local, regional, and national healthcare initiatives.

The Community Health Investment Program will support the services, programs, and agencies with strong, successful track records and the highest probability of meeting their defined goals and objectives.

Grants made by the Peninsula Health Care District must directly benefit those who reside in the District.  Agencies who provide services to individuals referred from outside the District, must be able to demonstrate that the use of grant resources meets, and are applied to, local needs.

The District will generally not consider requests for basic operating support, requests to cover budget deficits, or requests to support research projects.

Recipients are required to provide regular financial and operational reports and periodically may be requested to make presentations to the Community Health Investment Committee and/or the Board of Directors at a public meeting.
